Temporary Food Establishments

To operate a Temporary Food Establishment (TFE), both the TFE Coordinator Application and individual TFE Vendor Applications must be submitted in full at least 15 days prior to the event.

Required Details:

  • TFE Coordinator Application must include a complete list of participating vendors.

  • Individual TFE Vendor Applications require a $75 fee per vendor.

  • The Health Department will only accept vendor applications approved by the designated TFE Coordinator.


Applications submitted after the 15-day deadline or without coordinator approval will not be considered.


Mobile Food Unit or Push Cart Application Instructions

To ensure your application is complete, please include all of the following:

  1. Completed Application Form

  2. Proposed Menu

  3. Commissary Agreement Form (fully filled out and signed)

  4. Equipment Specification Sheets (for any appliances or food service equipment)

Note: A submission checklist is included in the Mobile Food Unit (MFU) application packet to help ensure your application is complete.

You have two options for submitting your MFU packet:

  1. Email Submission
    Submit the completed packet to: eh@gvph.org
    A GVPH staff member will contact you to collect payment by credit card.

  2. Mail Submission
    Mail the completed packet along with a check to the Environmental Health office in the county where your commissary is located.

 

Incomplete packets will delay processing. Please refer to the checklist before submission.

 Food Service Establishment Plan Review Submission Requirements
 

To submit a complete Plan Review Application for a Food Service Establishment, please ensure the following items are included:

  1. Completed Plan Review Application Form

  2. Proposed Menu

  3. Equipment Specification Sheets

  4. Site Plans or Building Plans (required for new facilities)

Note: If the application is for an existing facility undergoing a change of ownership, building plans may not be required. Please confirm with Environmental Health before submitting.

Incomplete applications may result in processing delays. Be sure to include all required materials before submission.
